You must make the appointment at least 24 hours in advance.

Please wear a facial covering in-office.

Our policy is to allow one parent or guardian with the child (please contact us if you require extra assistance).

All patients and families must wait in-office for 15 minutes after the vaccine so that we can watch for adverse effects.

Anyone with a history of allergic reactions to the vaccine or with a history of anaphylaxis must wait in-office for 30 minutes afterwards.

Please make sure all insurance information is correct and up to date.

When filling in the form, all information pertains to the child unless otherwise stated; e.g., the DOB refers the child's DOB, the name refers to the child's name, etc.

You do not have to be an Agave Pediatrics patient to book a COVID vaccine with us.
